Two weeks post-op

(English below)

It’s been just over two weeks since the last liposuction on my upper thighs. Recovery is going well. The blue, purple, red and yellow bruising is starting to fade quite a bit. The incision wounds are healing well. Fortunately, the rashes (intertrigo) are under control. I’m moving around fine. I’m already walking almost as much as usual. My upper thighs are still swollen and hard, though. They feel like a hard, thick slab. It doesn’t bother me much when I’m walking; in fact, that actually makes it a bit more supple. But when I bend my knees, it does pull and is painful.

Last night, the new line dancing season began. I’d said beforehand that I’d see if I could manage it. Well, if I can walk, I can dance too. Right? I took part in quite a few dances. Most of it went really well. It was just the turns where you have to bend your knees a bit that didn’t go so smoothly. That makes sense, but it doesn’t really matter. I had a lovely time dancing and I think it’ll go a bit better next time.

Last night I was finally able to take the compression garment off for the night. What a relief that is. Lovely. I slept fine with the compression garment (or sometimes the leggings) for those first two weeks, but it’s definitely much more comfortable without it.

Ron had to work this morning, so for the first time I got into the car myself to go to physio. Driving is fine fortunately, because I had to drive round in circles for 10 minutes before I found a parking space. It’s just getting in and out of the car that’s a bit trickier because of the bending.
>Maureen hadn’t heard of the LIPLEG study yet, so I sent it to her. She gave me a massage and, thankfully, things loosened up a bit again. We’ve scheduled two appointments a week for the time being.

Today was Friday – bookkeeping day – so I didn’t walk much. Well, 8,000 steps all the same from walking the dog, doing the shopping and popping to the chemist’s. As putting on the compression garment without Ron’s help is still painful because of my thighs, I’ve taken the day ‘off’ and kept the Lipoelastic leggings on. I’ll take them off before bed (yay!) and put the compression garment back on properly tomorrow.

Hell of a weekend

(English below)

Even lekker struinen is heerlijke afleiding.

Even relatieve rust… not!

Truste.

What a rubbish weekend! It all began on Friday, of course, with that rash (intertrigo). That was the start of a weekend full of stress. I put the compression garment back on during the day and then wore the leggings in the evening and at night. It’s manageable, and the rash has already improved a lot. It’s just that I’ve just spotted another red patch on the other side. Pff…

After doing the invoicing, I had a problem with my email programme. Of course, it was with my most-used account, that’s just how it goes. I spent the whole weekend trying to sort it out. I tried everything I knew and had ever heard of. In the end, I just threw it into Google AI and that brought the solution. It’s sorted now, but I still need to get to grips with it properly.

Ron’s stress levels had been building up quite a bit over the past week; he was really dreading tomorrow’s colonoscopy. Anyone who knows his background will understand why his reaction is so intense. He didn’t have to start taking the laxative until 6.00 pm today, so we thought: come on, let’s jump in the car and find a flea market. We always enjoy a nice market. A bit of a stroll there is a lovely distraction. We got in the car early this morning and… breakdown! Grr, back home empty-handed again. Ron checked the car thoroughly, because, well, we really did need it tomorrow! Luckily, we’ve still got some good contacts. The car’s being fixed.

So, email problem sorted, car’s being fixed. A bit of relative peace and quiet… not! My phone rang and I could see straight away it was the hospital. The gastroenterologist rang personally to say that, unfortunately, tomorrow’s procedure couldn’t go ahead. Two patients have been admitted with acute, life-threatening conditions who must be given priority tomorrow. So Ron’s appointment  has to be rescheduled. They were genuinely sorry about it and understood the impact, but there’s simply no other option. I was so angry and disappointed. Of course I understand the situation, but for Ron this is a real nightmare scenario: weeks of worrying again, the tension building up. It’s now up to me to arrange a new appointment with the outpatient clinic. Ron had also cancelled a long, nice work trip to make way for tomorrow’s exam, but that slot has, of course, long since been filled by someone else. Not the worst part of all this, but still a real bummer!

If anyone says one more time that you should minimise stress as much as possible because it’s bad for you, I won’t hold back. Go and tell that to the universe too, then! It felt like ‘F*ck Ron & Cora Weekend’. I even forgot this afternoon that the F1 Grand Prix was on!
All in all, it’s incredibly frustrating and so… no, not exactly helping my recovery. My thighs feel like concrete and they ache. They’re really tight, and I only feel any relief when I’m lying down. Hopefully they’ll ease up a bit later when I’m in bed. My body’s tired, and my mind’s had enough for now too. Tomorrow’s another day. We’ll see how things go then. Goodnight.

The LIPLEG study

(English below)

On 4 September 2026, the latest results of the LIPLEG study were finally published in *The Lancet*. This was a multicentre, randomised study conducted at 11 sites in Germany, involving 410 women with stage I, II or III lipoedema. Liposuction was compared with continued conservative treatment.

The primary endpoint of the study was a clinically relevant reduction in leg pain of at least 2 points on a scale of 0 to 10.
- 68.3% of the women in the liposuction group achieved this reduction in pain.
- In the conservative treatment group, this figure was 7.6%.
- The likelihood of a clinically relevant reduction in pain was therefore approximately 26 times greater following liposuction.

Liposuction also showed better results in terms of secondary outcomes. A significantly greater improvement in health-related quality of life was observed. In addition, improvements were seen in areas including physical health, mobility limitations and the tendency to develop haematomas. The positive effect was observed in women with stage I, II and III lipoedema.

This study thus demonstrates that the effect of liposuction relates not only to the removal of adipose tissue or changes in body circumference, but primarily to patient-relevant outcomes such as pain and quality of life. It is precisely these outcomes that are of great importance when assessing the clinical added value of a treatment.

At the same time, the study shows that liposuction is an invasive treatment. Adverse effects were more common in the surgical group: 47 per cent of participants who underwent surgery reported an adverse effect, compared with 21 per cent in the conservative group. Serious side effects were reported in 8% and 3% of participants, respectively. Most side effects were mild to moderate in severity, but serious complications were also observed, occurring exclusively in the surgical group. The researchers therefore emphasise that the benefits of liposuction must always be weighed against the surgical risks.

The LIPLEG study provides the first large-scale, randomised and controlled evidence that liposuction in women with lipoedema leads to a significantly greater reduction in pain and improvement in quality of life than conservative treatment alone. The long-term follow-up is still ongoing up to 36 months. These data are important for determining how sustainable the improvements are and what long-term risks and any late complications may arise.

In Germany, based on the available LIPLEG data, it was decided in 2025 to include liposuction for lipoedema in standard reimbursement under specified conditions. In the Netherlands, the Zorginstituut concluded in 2022 that liposuction for lipoedema did not meet current standards of scientific knowledge and clinical practice. The new LIPLEG results will hopefully provide an important new scientific basis for a future reassessment. I consider it unlikely that liposuction will be included in the basic healthcare package as it is in Germany, but it is a small step in the right direction.

Oh no, not again...

(English below)

Vandaag begon als een gewone dag. Goed geslapen, rustig opstarten.

Today started off as a normal day. I slept well and had a calm start to the day. I had breakfast and took my antibiotics. I got ready at a comfortable pace, then Ron drove me to Maureen’s for my first appointment since the operation. I told her how well things were going: no more leakage, the wounds already healed, no more plasters, and I was moving around fine. It was just still a bit tight due to the swelling, which is normal at this stage. No deep massage just yet though: it’s still a bit too painful for that. Meanwhile, she told me about her holiday, and I told her about the operation and other things. When she’d finished, I felt immediate relief: less pain, less tightness, more supple and, of course, I needed to go to the loo again. That’s a good sign: fluid is being drained! I took the compression garment home to wash it.

At home, I wanted to have a quick wash and change before putting the compression garment back on. However, I felt something on my tummy. I couldn’t see it properly, so I asked Ron for help. He immediately said, “Oh no…” but of course it was “Yes, indeed”. A large patch of rash (intertrigo) below my tummy. Yuck. Sigh. No panic this time. Well, a bit, but mostly anger, worry and a sense of powerlessness. My bloody skin’s messed things up yet again. It’s literally and figuratively wearing me out! I could have cried, and I did for a moment.

But then again, what’s the right thing to do…? Should I put something on it? Pressure suit or not? I took some photos and sent them to Joost. I’d promised Maureen I’d email her the surgery report, so I told her about it too. After that, we put some ointment on it and placed a piece of gauze in the skin fold. In the end, I decided to give the clinic a quick call after all, as Joost doesn’t always have time to keep an eye on his WhatsApp. I got Marco on the phone. Joost was with a patient, but he’d pass on that he needed to have a look at the photos in the app.

In the meantime, I had to get to the bookkeeping. The invoices go out on Fridays. Hmm… sitting at my desk means keeping my legs down. Without the compression garment, that’s not ideal at the moment because of the fluid and the swelling. But then again, extreme pressure on the affected area isn’t good either. In the end, I decided to put on the Lipoeastic leggings as a temporary solution: they provide compression but not too much pressure. That worked out fine. I found it really hard to concentrate, but I’d already prepared everything, so it didn’t take too long before I was finished.

Joost didn’t ring or text, which was a bit of a let-down. I’m so spoilt having a doctor who’s accessible and on standby. But whilst I normally don’t mind if he can’t reply straight away (after all, then it’s not urgent or anything), this time I was a bit disappointed. I didn’t ring for no reason. I never do that unless it’s absolutely necessary. Perhaps I should have been clearer about that, mind you – he’s not a psychic or a Cora whisperer. I’ll have a chat with him about it. I was just too emotional at the time.

I was really upset for a moment and had a proper cry. Luckily, there was a lovely distraction this afternoon: the neighbours came round to visit. I’d looked after their cats for a few days and, as a thank-you, I was given a really cool mug: a black one with a cat and ‘The Witch Is In’ on it. And we were given some homemade chocolate chip biscuits: yum! We had a lovely chat about all sorts of things. That was exactly what I needed.

After their visit, I saw that Maureen had replied to my email. She confirmed that I’d done the right thing. I thought so myself, but it’s still nice to hear. I sent Joost another quick text, asking if it would be a problem if I slept without the pressure suit tonight. That’ll give the area a bit of a rest, but it’s actually still too early and I don’t want to jeopardise my recovery. Early in the evening, he replied with a detailed message. I’d handled it well indeed. No pressure suit tonight, and we’ll see how things look tomorrow, depending on how the area looks then. That did put my mind at rest for a bit. Still, it’s been a crappy evening. I’m tired and overstimulated, so I’ll wait until I’ve taken the last antibiotic of the day and then I’ll go to bed.

I’m sure you’re not keen on seeing a photo of an ugly blemish, so here’s my new mug!

Full speed ahead

(English below)

Keira

Phew, what a day… From couch potato to full steam ahead. After yesterday’s off day, I really wanted to get moving again today, but I ended up doing nearly 10,000 steps. Normally, that’s no problem at all for me, but on the fourth day after the operation, it was perhaps a bit too much. Mind you, apart from feeling tired, I haven’t had any pain or discomfort, so that’s actually quite a good thing.

This afternoon I did indeed go to the Recovery Academy; lovely someone offered to pick me up and bring me home. But stupid, stupid, stupid: I’d forgotten my pee funnel! I’ll have to get used to taking it with me again. I didn’t fancy having to take the whole compression suit off just to go for a wee, so I stuck to just one cup of tea. It was nice to get out for a bit and chat with the group. They were quite surprised (and pleased) that I was able to be there. Me too, haha!

When I got home, Ron was just about to walk to the supermarket, so I went along. That went fine. I also popped into Kruidvat to get some paracetamol. After dinner, I wanted to try walking Keira on my own. She’s always boisterous, but this time she seemed to sense perfectly well that she shouldn’t pull too hard. She walked calmly with me down the stairs to De Geul. I unleashed her for a moment so she could let off some steam, and afterwards she came back nicely on the lead.

I was really tired, though, so I spent the rest of the evening on the sofa with my legs up. With the cat on my lap, which is fine as long as she doesn’t stretch or move: claws! But at one point, Keira thought, “How cosy, I’ll come and lie down with you!”, and rammed her shoulder right into my thigh. I was knocked sideways… She was startled by my loud “Ouch!” Of course, I didn’t punish her; she doesn’t know any better, she didn’t do it on purpose. But ugh, that really hurt and it’s still sore. Probably another bruise tomorrow.

Day 3, bleh

(English below)

Vandaag was een beetje een off-day. Niets ernstigs, gewoon mega-bleh. Bankhangdag.

Gewoon even eruit, de sleur doorbreken. We’ll see. Mentaal broodnodig.

Today was a bit of an off-day. Nothing serious, just really blah. That’s all part of recovery, but it’s not exactly pleasant. I’d slept well, but I wasn’t feeling too great. Not ill, just a bit low. Tired, no appetite, no motivation. A day spent lounging on the sofa. That’s allowed, isn’t it? Hopefully I’ll feel a bit better tomorrow.

The recovery itself is going quite well. It’s fairly blue, with some darker patches. I’m not oozing at all anymore; the small incision wounds have closed. We’re still putting plasters on them just to be on the safe side, so that the compression garment doesn’t rub the scabs off. My whole upper legs are swollen and feel tight, but it’s manageable. So far, I’ve been out for a short walk every day. Some days it goes more smoothly than others, but I do it anyway. Even on this blah day. Not pushing myself too hard, but still persevering.

Boredom is certainly setting in. My head’s too foggy to read, and I can’t last long doing things at my desk or at the table. So, lots of telly, but that also wears me out. So I’ve planned an ‘outing’ for tomorrow. We’ve got a meeting of the Eating Disorders Recovery Group that I facilitate. I’d already said I couldn’t make it, but I’m going anyway! Someone else is coming from IJmuiden; they’ll pick me up and bring me home. Just a little change of scene, to break the rut. They’re always lovely meetings, where I can share my story just as well. A smart move? We’ll see. Mentally, I really need it. Physically, it might be a bit of a challenge, but nothing I can’t handle, I reckon. I can put my legs up there too; I’ll rest up when I get home.

Recovery Day 1

 (English below)

Mijn behandelaars mogen in principe delen/gebruiken.

Heerlijk even naar buiten!

The first day. I was feeling confident about the procedure itself, but I’m finding the first few days a bit of a struggle. I actually slept quite well, but then again I was absolutely knackered after yesterday’s intense day. The compression suit was getting tighter and tighter, and the pads were getting heavy with moisture. The tight bodysuit was making my breathing shallow, so that was the end of size L.
When it really got too much, we went upstairs to unwrap me and rinse my legs. The pads were indeed absolutely soaked with fluid – what a weight! Ron took some photos to document the progress. Joost had sent the discharge letter yesterday and asked me to let him know today how things were going. So I sent him a photo and a message.

In the end, it turned out that only one incision was still leaking quite a lot. So we put a pad on that one, and applied island plasters to the rest. We just used the XL compression suit from last time, which gave (literally) a bit more breathing space. Putting it on is still a nightmare. I’m already looking forward to being rid of the compression garment. Just got to hang in there for another six weeks or so, phew… Well, as long as I can take it off at night first; in theory, that should be after two weeks.
The bruising wasn’t as bad as I’d expected (yet?). It’s particularly discoloured around my knees. The lipoedema fat is very stubborn there, so that area will be treated more than once. The rest is mainly very swollen and feels sore. It’s also a bit numb here and there. I can touch it, but I can’t press on it. My upper legs feel tight and thick. Walking is a bit easier without the extra weight, though.

I’ve included a photo here, despite my swollen legs caused by the fluid retention. I believe in honest photos. Liposuction for lipoedema is not cosmetic surgery to achieve beautiful legs (are you listening, Dutch Healthcare Institute?!). And yet you often see ‘before and after’ photos at clinics, where I’d practically do anything to look like the ‘before’ photo, so to speak. Mind you, I’m not trivialising anything! Even with beautiful, super-slim legs, you can suffer a great deal of discomfort and pain from lipoedema. Still, I want to show exactly what it looks like at my size. So… an honest photo.
Just to be clear: I’m posting these photos publicly on my blog to reach as many people as possible, but I’m not waiving my rights! Feel free to share them, but please credit the source and include the accompanying information. My express prior consent is required for commercial use. In principle, my doctors are allowed to share or use whatever I post.

We didn’t do much today. I’m still very tired. Still, I did try to move about regularly. When Ron took Keira out for a walk at the end of the afternoon, I pushed through my tiredness and went along. It was lovely to get out for a bit! Luckily, it has cooled down a bit. I’m still a bit unsteady now and then, and the stairs down to De Geul were quite a challenge, but I managed it. Off we go! Back home on the sofa I put my legs up, and I promptly fell asleep, haha. Later on, I’ll install myself on the sofa again and hope to join in the Full Moon celebration of the Temple of Ara for a bit. That’s not until 1 o’clock (7 pm New York time), so I’ll see if I can manage it.

I’ve taken the first step. Here’s to a speedy recovery.

Liposuction, part 3

 (English below)

Today was the day: the third of four procedures, the last one on my thighs, the front. As I’ve come to expect by now, I was both looking forward to it and dreading it. I was due to arrive at the Noordveld Clinics at 8.30 am. The roads were busy, but we arrived well on time. Ron went home again to sort everything out there, whilst Joke stayed with me. I like the idea of having someone nearby. Although I certainly also feel supported by all the messages, texts, etc. from all of you. Whilst the doctors were working, I was able to meditate now and then, and I could almost physically feel the support behind me. Carried by the community, just like in my shamanic journey last Sunday.

The team had changed slightly. Of course, Joost was still the lead doctor and Marco was handling the anaesthesia. As I was there on a different day, it wasn’t Tieneke but Paul – just as skilled and a really nice bloke too. He’s American and is trying to learn Dutch, which is always nice. Joost asked if another doctor could observe the whole process. I didn’t have a problem with that. If Joost trusts him, I trust Joost. He asked for permission politely before doing anything.

The atmosphere was the same as before. Total focus when needed, but in between there was just a bit of chit-chat about all sorts of things. ‘Cosy’ might be a bit of an exaggeration, but I felt completely at ease. That’s worth so much! I had the undivided attention of four lovely men for hours – what more could I want? LOL

First, I changed into the super-charming (ahem…) surgical gown, then they marked me up. This was filmed for educational purposes, again with my consent, of course. They took my blood pressure (which was fine), started me on the antibiotics and paracetamol, and then it was time for Marco’s Magic, assisted by Paul. The first few needles are always a bit of a nuisance; not painful, just little pricks. Once the anaesthetic kicks in, it becomes increasingly numb. I can still feel the pressure and that they’re doing something, but without any pain. My knees and groin remained a bit sensitive, so a bit more was used there. In the end, 9 litres of anaesthetic fluid. After a period of rest, my pee funnel will be working overtime again!

I had to wait for about three quarters of an hour for the anaesthetic  to take effect. I let a few people know what was happening, replied to some messages, posted a photo and then meditated. It’s good for the heart rate and just a lovely way to relax.

The liposuction itself went pretty much the same as the previous times. Joost had another proper workout out of it. Every now and then he really put his back into it. I didn’t mind – off with that wretched fat. It was a bit sensitive at times, but it was manageable. Because he was explaining things and Patrick was asking questions, I had a good understanding of what was happening. Interesting and informative. Patrick also did part of it to get a feel for the cannula. He did it well and carefully, under Joost’s supervision. In the end, it took just over an hour and a half.

The only thing that was a bit uncomfortable was having to lie on my back for so long. On other occasions, I’d sometimes have to stand up or lie on my side for a bit. Not this time, though. Every now and then, a pillow was placed under my lower legs to create the right angle, which was nice. If I mentioned it, I could arch my back a little and then it was fine again.

During the liposuction, Paul had already squeezed a lot of the excess fluid out of my other leg. It’s a bit of a strange sight – there are actually little fountains gushing out of the incisions! But then again, whatever’s already come out doesn’t have to be drained internally by my body.

I was wrapped up in ‘nappies’ and absorbent pads. On top of that came the compression garment. An L this time – one size smaller! That always makes me do a little mental dance. If the swelling gets too bad over the next few days, I can temporarily use the XL from last time.

I got dressed; Joke helped me with my shoes. In the waiting room, Marco gave me a lovely sandwich. Joost and Patrick popped in for the debrief, and then Ron was at the door.

Back home, I settled down on the sofa. Joke had a drink here and then I had a quick nap. I took the next dose of antibiotics and paracetamol. I’ve started using arnica drops again for the bruises. On previous occasions, they really did make a significant difference to the discolouration. Now I’m in relaxation mode with a cat on my lap!

Three nights until the third lipo

(English below)

Afijn, druk druk druk. What’s new?

Alvast bedankt!

Card from the Witches' Wisdom Tarot by Phyllis Curott & Danielle Barlow

Just three more nights to go until the third liposuction. And, as always, things can’t just go smoothly for us. Last week, the back of my front tooth suddenly broke off, so that meant two more dentist’s appointments. Today, Ron had to go back to the pain clinic for more injections in his back, in the hope that they’ll last longer this time. In early September, he’s also due for a colonoscopy, for which I’m now making all the necessary preparations. Meanwhile, we’re planning a bit of home improvement: finally a new window frame in the bedroom and a door to the roof terrace. Not to mention all the ‘normal’ things that take up time. Anyway, busy, busy, busy. What’s new?

I’m making the final preparations. I’ve filled in the questionnaires from the clinic. I’ve transferred money from the special savings account. Now I just have to wait for it to appear in my current account, then adjust the daily limit, wait four hours and make the transfer. Just a few final things to check and sort out… I’m almost ready. Being busy can be a lot at times, but it also takes my mind off things. That way, I can’t get too worked up. Do I get too worried then? Hmm, well, a bit, I suppose. I’m not scared, mind you; I know I’m in good hands. Unfortunately, the weather’s set to be tropical this coming Thursday, yuck. Luckily, it looks like it’ll be a bit better after that. I’m a bit daunted by the leakage during the first few days, and by the pressure suit. Oh well, I know it’ll all be over soon. And then my thighs will be done, which is a nice thought already!

Last weekend, the theme of our Witches’ Wisdom Circle and the Journey Circle was: “the power of community connections”. They were lovely gatherings with ritual and a journey. We talked about the power of a community: those around you in the neighbourhood, at work, through a hobby, fellow sufferers when ill, and of course the Temple of Ara / Phyllis Curott community. That’s very important to me (and many others).
In my weekly reading for the Witches’ Wisdom group on Facebook, the “Healing” card appeared. Not surprisingly, that had also been the case before the former procedures. It’s a group reading where I draw three cards and everyone chooses one for themselves, but at the same time, the cards also convey their own message to me personally as a three-card spread. Clearly, one doesn’t rule out the other. That’s the power of tarot. Interestingly, on this card you can also see a circle of people.
During my shamanic journey, I met Chiron (the wounded healer) again and swam with dolphins and orcas. When I grew tired, they carried me further through the water whilst I rested. Literally carried by the community. The journey showed me reality in a symbolic way. This whole process of lipoedema reduction surgery is, of course, very personal and I have to do most of it myself, but feeling supported is worth so much. I feel the connection. And that community…? That’s you, reading this. All of you, in whatever way you can.

So yes, I’m just going to ask again: will you spare a thought for me on Thursday morning? Light a candle, say a prayer, send some reiki, perform a proper ritual, or just think of me for a moment and wish me well. Do whatever feels right for you with the right intention – anything goes. Thanks in advance!

Compression leggings and an update

(English below)

Haven’t written for a while, how time flies. No news is good news? Yes, as far as my legs are concerned certainly! Things are going well. I can really feel a very clear difference in terms of pain. The chronic pain is clearly concentrated in the areas that haven’t been treated yet: the front of my thighs and my lower legs. I can stand for longer. For example, I don’t always have to use the shower stool when I’m having a shower anymore. And last night I stood in a queue for nearly an hour to get a pair of eclipse glasses, and that went well! Well, the standing part anyway, the glasses were sold out before we even reached the counter.
I can also feel a difference when touched. That always used to hurt, no matter what. However, I can now simply touch the treated areas without any pain – it’s such a strange sensation… I thought I could feel a really hard lump in my thigh, but it turned out to be my muscle, LOL! I’d never been able to touch it so directly before.

I do, however, still have some really hard lumps here and there, particularly where the most lipoedema fat was removed. That’s why, once I was allowed to take the compression suit off, I decided to go for a pair of compression leggings instead. Not those awful things I was fitted with – putting those on took more energy than I had. I now have two pairs of Lipoelastic leggings: the ACTIVE model, because it offers medical-grade compression and comes in a plus size for lipoedema calves. I’m really happy with them. I might need a smaller size once everything’s over, but we’ll see about that later. For now, this is a great solution. I just wear them as trousers with a shirt over the top. They’re nearly 80 euros each, but they’re definitely worth the money. And just to be clear: this isn’t a sponsored post! I just like to share things I’m happy with.
For comparison, the only photo I could find was from 2021 of me wearing the custom-fitted compression capri and stockings. There’s also a difference of nearly 20 kilos, so it’s not a proper before-and-after, but do take a look at the contours of my legs. I promise to compare my bare legs properly later on, but I prefer the look of the trousers. :)

The third surgery is slowly drawing nearer, just two weeks to go (Thursday 27 August). We’re now in the midst of yet another of those dreaded heatwaves; it feels as though there’s no end to them this year. Will you keep your fingers crossed for cooler weather after my operation? Because then I’ll have to wear the compression garment day and night for another two weeks, and then for several more weeks during the day.
As for the procedure itself, I’m both looking forward to it and dreading it. The upside is that I now have a bit of an idea of what to expect. This time it’s the front, the lower part of my thighs. At least it’s in sight, which I hope will make dressing the wounds and so on a bit easier. The post-operative treatments are already scheduled. Maureen is on holiday at the moment, but she’ll be back exactly the week I need her again. That’s what I call good timing, isn’t it?

Since my last post, I’ve received some more donations, and some more today as well. Thank you so much for all your support. Not just the financial support, however welcome that is, but also the messages, cards, requests for updates, and so on. I really feel supported, and during such a long process, that’s worth its weight in gold. You’re all amazing – big hugs!

Same BMI ≠ same biology

🧬 SCIENCE IS CATCHING UP: YOUR BMI DOESN’T TELL THE WHOLE STORY

What if two women have a similar BMI—but their bodies are not metabolically behaving the same way?

A new study published in Frontiers in Endocrinology found distinct metabolic patterns in women with lipedema compared with women who had a similar BMI but did not have lipedema.

Researchers studied 24 premenopausal women with lipedema and 21 BMI-matched controls. And they found something important.
🔬 Women with lipedema had:
• Lower fasting insulin
• Lower HOMA-IR, a measure used to estimate insulin resistance
• Lower levels of several metabolites involved in glycolysis, including lactate and pyruvate
• Higher levels of ketone bodies
• A distinctly different pattern of fat distribution

Here’s what caught my attention: The women with lipedema had similar BMI—but their metabolic profile was not the same. That matters. Because BMI tells us how much someone weighs in relation to their height. It does NOT tell us:
❌ Where that fat is distributed
❌ What type of tissue is contributing to the weight
❌ How that tissue behaves
❌ How the body is processing energy
❌ Whether two people with the same BMI have the same metabolic biology

The researchers used something especially interesting called composite metabolic indices. Instead of looking at one metabolite at a time, they looked at groups of metabolites representing biological pathways. And when they did that, the metabolic differences between the lipedema and control groups became much clearer.

⚠️ But let’s be scientifically honest. This was a small study—only 45 women total—and it was conducted at one point in time.
It does NOT prove that these metabolic differences cause lipedema.
It does NOT give us a blood test that can diagnose lipedema.
And it does NOT prove that a particular diet or supplement will treat lipedema.

But it does add another important piece to the puzzle. Lipedema may not simply be “obesity in the legs.” The biology is beginning to tell us something different. And perhaps one day, instead of asking: “What does she weigh?” we will also ask: “What is her body doing?”
That is the kind of research I want to keep watching. Because science is catching up. 🧬
